Healthy Smiles Ontario (HSO)
Healthy Smiles Ontario (HSO) is a dental care program that covers the cost of regular check-up, cleaning, scaling, filling and X-rays.
It does not cover the cost of braces, space maintainers, cosmetic dentistry, teeth whitening.
Eligibility
- Your child must live in Ottawa and be 17 years old or less
- Your household adjusted family net income must be $20,000 per year or less
- Your child does not have access to any form of dental coverage
How to apply
Download the Healthy Smiles Ontario Program application form or call Ottawa Public Health at 613-580-6744 to obtain the form.
Complete, sign and take your application form with the required documentation to any Client Service Centre.
Documentation you need to bring to the client service center with your application
- Proof of income in the most current tax year
Annual Canada Child Tax Benefit and Ontario Child Benefit Notice
OR
Annual Goods and Services Tax Credit Entitlement Notice showing the adjusted family net income $20,000 or less
OR
Ontario Sales Tax Credit
Note: Financial documentation must list name of each child being enrolled
- Parent/Guardian - Proof of identification: (show one of the following):
- Valid Ontario Health Card (with photo)
- Valid passport
- Certificate of Canadian Citizenship (Citizenship Card)
- Permanent Resident Card
- Canadian Immigration Identification Card
- Valid Ontario Driver’s License
- Child - Identification required for each child, acceptable identification includes:
A valid Ontario Health Card (with birth date)
If a dependent’s Ontario health card is not available, show one of the following:
- Valid passport
- Certificate of Canadian Citizenship (Citizenship Card)
- Permanent Resident Card
- Canadian Immigration Identification Card
- Birth Certificate as issued by a Canadian provincial or territorial government
- Valid Ontario Driver’s License
AND one of the following to confirm residency in Ontario
- Rental or lease agreement
- Mortgage papers
- Utility bill
Note: two signatures are needed, if your marital status is married or common law.
The application will be forwarded to Ottawa Public Health for final determination of eligibility. Healthy Smiles Dental Cards will be mailed within seven to 10 days.
Children in Need of Treatment (CINOT)
Children in Need of Treatment (CINOT) is a limited emergency dental care program. If your child has dental pain, an infection, cavities or a mouth injury, treatment such as radiographs, fillings, or some other dental care may be available.
Eligibility
- Your child has a dental emergency/pain
- Your child must live in Ottawa and be 17 years old or less
- Your child does not have access to any form of dental coverage
- You cannot afford the cost of urgent dental care
How to apply
Your child can be screened at one of the following Ottawa Public Health Dental Clinics from 8:30 am to 4:30 pm, closed between noon and 1 pm.

Free dental screenings, fluoride varnish applications and dental health information are available in the community. Please refer to the monthly calendar for the closest location.
