ServiceOntario offers a comprehensive online Newborn Registration Service to simplify the process of registering your new baby. In addition to registering your baby’s birth, you can use the service to apply for your baby’s birth certificate and Social Insurance Number and to apply for Canada Child Benefits.

ServiceOntario still offers a paper-based registration service for parents. Complete the Statement of Live Birth form, print it and mail it to the following address within 30 days of the birth of your child:

Travelling with a newborn

The City of Ottawa does not issue Temporary Confirmation of Birth Letters (Travel Letters) for new births Parents travelling outside of Canada who need birth confirmation documents urgently must contact the Office of the Registrar General at 1-800-461-2156 to check the status of their child’s birth registration.

Birth certificate application 

The Office of the Registrar General of Ontario processes birth certificate applications. If you didn’t request a birth certificate during the online registration process, you can file a separate request through with the online Birth Certificate Application.
