Request an investigation under the Municipal Conflict of Interest Act

An eligible elector or a person demonstrably acting in the public interest who believes a Member of Council or a member of a local board has contravened the conflict of interest rules of the Municipal Conflict of Interest Act may apply to the Integrity Commissioner to request an investigation.

Before you begin

Note

  • There is no cost to file a request for investigation.
  • A request for investigation must be filed within six weeks of becoming aware of the potential conflict of interest.
  • A request for investigation can be filed by:
    • an eligible elector; or
    • a person demonstrably acting in the public interest.
  • Anonymous inquires will not be accepted.
  • All requests will be treated as confidential at all times.

What you need

  • A completed Application for Inquiry/Affidavit, which includes a signed and sworn affidavit. Note: A paper copy can be requested from the Office of the Integrity Commissioner or by calling 613-580-2400 ext. 21978, TTY 613-580-2401.
  • The details to support the allegation made against a Member including date(s), time(s) and location(s) of the conduct, name(s) of person(s) present and/or involved, and all other relevant information.

The Application for Inquiry/Affidavit and supporting documentation can be sent by email to integrity@ottawa.ca. The email must include a copy of the completed form, including a sworn affidavit, and all supporting documents.

What's next

Once your form and any accompanying documentation has been received by the Office of the Integrity Commissioner, the Office will contact you to confirm receipt, request any missing information, and explain next steps.
