Renewable Recreation Funding Program

The Recreation Funding Unit invests in non-profit based organizations that support recreation programming in the City of Ottawa.

Only organizations that are currently receiving Recreation Renewable Funding should complete the 2022 Funding Submission Report form.

Deadline: Monday, January 31, 2022, 4 pm.

Please note: You must download and save the form onto your computer and then open it with Adobe Acrobat Reader to complete it. The form allows users to enter and save information. There are some fields that will auto-calculate and self-populate.

Any information added to the form, prior to saving it to your computer, through a web browser will be lost when the form is downloaded, printed, or saved.

Only organizations that are currently receiving Recreation Renewable Funding can complete the 2022 Funding Submission Report form. For 2022, the Funding Submission has to be completed and received by the Recreation Funding Unit no later than Monday, January 31, 2022, 4 pm.

Completion of all required sections including

  • Two signatures from individuals who have signing authority for the organisation on page 18 - Signature Block. Electronic signatures are accepted.
  • Details about your Reserve Fund(s) on page 17.
  • Financial Information on pages 11 to 16 (2021 revenues/expenditures and 2022 forecasts).

Audited financial statements – for last completed fiscal year

  • Consult your Contribution Agreement Schedule A - Annual Reporting Requirements, for organisations that may submit financial statements that have not been prepared by a “qualified” auditor (unaudited financial statements).
  • Unaudited Financial Statements should include annual revenues, expenditures and highlight any yearly surplus, deficit and ongoing reserve fund.

Annual report – for last completed fiscal year

  • In the absence of a formal Annual Report, please attach a summary that includes your organisation’s activities and accomplishments in 2021, and goals for 2022

Annual general meeting minutes – for the last two completed fiscal years

  • Most recent approved Annual general meeting minutes signed by two individuals with signing authority for your organisation. Electronic signatures are accepted.

Current board of directors list

  • Include the names, position, address, contact information and the number of years each member has been on your Board

Valid certificate of insurance 

  • Identifying the City of Ottawa as an additional insured. Ensure insurance coverage is adequate as stipulated in your legal agreement.

Please complete the 2022 Funding Submission Report form, and submit via options 1 or 2 as described below. For 2022, the Funding Submission has to be completed and received by the Recreation Funding Unit no later than Monday, January 31, 2022, 4 pm. 

Due to COVID-19, in person drop-off of the Funding Submission and attachments is not available.

Option 1 (Preferred): You may send us a link to download your Submission and attachments through an online file storage site such as Google Drive, Dropbox, etc. All submitted documents must be in a minimum font of 12 points. Clearly identify your organization name and ensure that permission to download has been provided.

Option 2: You may email the completed submission to rec-info@ottawa.ca. Please ensure that the subject line contains the name of your organization, e.g.; ABC Company - 2022 Funding Submission. The total size limit for incoming email attachments is 15 MB. Please try to send in one email if possible, however, if it is too large please send two emails and use the subject line ABC Company - 2022 Funding Submission Part 2. It is your responsibility to ensure that the document is sent. You will receive an email that the document has been received by our inbox.
