Please submit one cleanup report for each project you completed by June 15, 2024.  You will require the registration code to submit this report. The registration code is a five-digit number located at the top of your registration confirmation email.

Students will receive a confirmation of their community involvement hours via email by June 30, 2024.

Personal information collected on this form is for the proper administration of the Civic Pride Program, as approved by City Council on February 16, 1994, and established pursuant to subsection 10(1) of the Municipal Act, 2001 S.O. 2001, Chapter 25, and will only be used for registration, planning, and tracking participants in the Cleaning the Capital campaign.
