Anyone who identifies or witnesses behaviour or activity they believe to be in violation of one of the three codes of conduct may file a complaint.

- The Integrity Commissioner does not receive complaints about City staff with respect to alleged non-compliance with the Employee Code of Conduct.
- Determine which Code your complaint falls under (who is the complaint or allegation about):
- A Member of Council - Code of Conduct for Members of Council
- A citizen member of the Built Heritage Standing Committee - Code of Conduct for Citizen Members of the Built Heritage Standing Committee
- A member of a local board - Code of Conduct for Members of Local Boards
- Review the information regarding the codes of conduct and Frequently Asked Questions.
- Review the relevant Code of Conduct and determine how you wish to proceed with your complaint, either under the informal complaint procedure or formal complaint procedure.
- Seek clarification or send any questions you may have to integrity@ottawa.ca.

- There is no cost to file a complaint.
- Anonymous inquires will not be accepted.
- All requests will be treated as confidential at all times.

Individuals are encouraged to use the informal complaint procedure as the first means of remedying behaviour or an activity they believe violates a code of conduct.
To file a formal complaint with the Office of the Integrity Commissioner
- Individuals must provide a completed Request for investigation form, which includes a signed and sworn affidavit. Note: A paper copy can be requested from the Office of the Integrity Commissioner or by calling 613-580-2400 ext. 21978, TTY 613-580-2401.
- The form must be signed by a Commissioner of Oaths who will ask you to swear or affirm that what is in the document is true. This service may be provided by a lawyer or paralegal, or other persons regulated under the Commissioners for Taking Affidavits Act.
- Commissioner of Oaths’ services are offered in person at four Ottawa Client Service Centres by appointment only. More information on the Commissioner of Oath services is available on the City of Ottawa website.
