Anyone wishing to question the appropriateness of a closed meeting of City Council, a local board, or a committee may file a request for investigation.

- Confirm if the meeting that was closed in full or in part to the public involved one of the following bodies:
- City Council
- Standing Committee, Sub-Committee or Transit Commission
- Advisory Committee
- Local Board (excluding Conservation Authorities, Police Services Boards, School Boards and Public Library Boards).
- Review the information regarding the role of the Meetings Investigator and the rules about open and closed meetings.
- Seek clarification or send any questions you may have to integrity@ottawa.ca.

- There is no cost to file a request for investigation.
- Anonymous inquiries will not be accepted.
- All requests will be treated as confidential at all times.

Once your form and any accompanying documentation has been received by the Office of the Integrity Commissioner, the Office will contact you to confirm receipt, request any missing information, and explain next steps.
